Overview
Chronic obstructive pulmonary disease (COPD) is a progressive lung disease where the airways and alveoli are damaged enough that air can't move in and out normally. In high-income countries tobacco smoking is the main driver, while globally indoor biomass smoke (like cooking over open fires) and workplace exposures are huge contributors too.
Impact
COPD is a leading cause of death and disability worldwide, and day to day it wears patients down with breathlessness, cough, fatigue, and flare-ups that often land them in the hospital. It travels with heart disease, lung cancer, osteoporosis, depression, and anxiety, and as the disease progresses, caregivers take on a lot of the emotional and logistical weight.
Medical Overview
Diagnosis requires post-bronchodilator spirometry showing a FEV1/FVC ratio below 0.70, and severity gets staged using FEV1, symptom scores (mMRC or CAT), and how often a patient has flare-ups, all of which is laid out in the annual GOLD reports. Core management is smoking cessation, staying up to date on vaccines (flu, pneumococcal, COVID-19, RSV when appropriate), pulmonary rehabilitation, and stepwise inhaled therapy with long-acting bronchodilators (LABA and LAMA). Inhaled corticosteroids are added for patients with exacerbations and eosinophilic inflammation. Flare-ups (often triggered by viral or bacterial infections or air pollution) are treated with short-acting bronchodilators, systemic steroids, sometimes antibiotics, and noninvasive ventilation when needed. Managing the other conditions that travel with COPD (heart disease, anxiety and depression, osteoporosis) is critical because they strongly shape outcomes. For advanced disease, options include long-term oxygen, lung volume reduction (surgical or endoscopic), and lung transplantation in carefully selected patients.
